The Rock Island High School Athletic Department is offering free sports physicals for the 2020-2021 school year to Jr. High students on Thursday, August 20.

Sports physicals for Edison Jr. High students will be from 6:00 P.M. - 7:00 P.M. and 7:00 P.M - 8:00 P.M. for Washington Jr. High Students. 

Physical forms (attached below) must have parent signatures in order for a Doctor to complete. 

In order to offer the free physicals we must keep our numbers to a minimum. Parents will not be allowed to walk through the physical process with the athletes. We understand if you do not approve with this request. You will need to make other arrangements to complete the physical if this process is not acceptable to you.
